Pacific changes vendor for voluntary life insurance

Effective Jan. 1, 2018, the vendor for this coverage will change from Sun Life to Liberty Mutual.

With this change in vendors, there is a one-time special enrollment period allowing you the opportunity to purchase additional Voluntary Term Life and AD&D insurance for you and your family. You may buy up to the guarantee issue amounts for any existing enrollees or new enrollees during Open Enrollment with no Evidence of Insurability (EOI). The guarantee issue amount for employees is one times the annual base salary, and for spouse and/or child(ren) is $20,000. Above those guarantee amounts, evidence of insurability is required before Liberty Mutual underwriters will approve new coverage amounts. 

As a reminder, employee coverage is available for up to five times employee's base salary not to exceed the maximum coverage amount of $1,000,000. Spouse/Domestic Partner coverage is available in increments of $10,000 up to a maximum coverage amount of $50,000. Coverage for children is available for $5,000, $10,000, or $20,000 per child. The premium is the same regardless of the number of eligible children covered.

If you would like to enroll in or make a change to your Voluntary Life Insurance during Open Enrollment 2018, simply complete an Enrollment/Change Form and be sure to check the appropriate boxes in the Voluntary Life section. If you plan to keep your coverage the same, you do not need to do anything. Your exsiting coverage will be transferred as of Jan. 1.

Open Enrollment is from Oct. 30 to Nov. 28.  All changes must be submitted to Human Resources by Nov. 28 and will be effective Jan. 1, 2018.        

This is also a great time to update your beneficiaries if you have not done so recently. Simply complete a Beneficiary Designation Form and turn it in to Human Resources. Beneficiaries can be updated at any time.
