New Hire Benefit Information and Forms

Listed below are forms which need to be completed and returned for your personnel file. Please note the dates upon which forms should be returned.

Staff Handbook Acknowledgment Form  - Return to Human Resources within 7 days. 

Sexual Harassment Acknowledgement Form   - Return to Human Resources within 7 days. 

Confidentiality Form  - Return to Human Resources within 7 days. 

Acknowledgement (Medical Plans)  - Please return to Human Resources within 7 days.--**In progress**

Benefits Enrollment Form  - Return to Human Resources with 31 days of hire.

Beneficiary Form (Life Insurance only)  - Return to Human Resources with Benefit enrollment form.

FERPA Instructions  - Complete within the first 2 weeks.

Direct Deposit Form  - Return to Payroll office.

Conflict of Interest Form - Return to Office of Internal Audit and Compliance.

Retirement Contact TIAA (enroll in Supplement or Group Plan).
